Implementation of New COVID-19 Tracking Forms
Kepada Guru & Staf Administrasi PBS yang terhormat, Pandemi COVID-19 diperkirakan masih akan terus berdampak pada komunitas sekolah selama beberapa bulan kedepan. Meskipun pandemiCOVID-19 akan selalu naik dan turun, PBS berkomitmen untuk menjaga kesehatan dan keselamatan siswa, guru, dan staf. Kami menerapkan protokol Kesehatan yang sangat ketat. Tim Penanggulangan Covid-19 PBS menerapkan dua cara untuk melacak kasus COVID-19 yang terkonfirmasi dan kemungkinan individu yang terpapar COVID-19 di dalam komunitas PBS.
Pertama, komunitas PBS dapat mengakses dokumen di bagian Formulir situs web COVID-19 PBS.
Guru, staf, dan siswa yang telah terkonfirmasi COVID-19 harus melaporkan hasil SWAB ke sekolah. Selain itu, guru, staf, atau siswa yang terpapar juga harus melaporkaninformasi ke sekolah.
Kedua, setelah selesai mengisi formulir, petugas kesehatan dari Puskesmas akan diberi informasi, informasi tersebut akan dialihkan ke guru atau staf PBS dan proses penelusuran kontak erat akan dimulai. Individu yang mungkin telah terpapar akan diberitahu secara langsung.
Guru atau staf Komunitas PBS yang terkonfirmasi positif atau terpapar virus COVID-19 harus mematuhi protokol Kesehatan Sekolah sebagai berikut:
Perjalanan Pribadi, Domestik, dan Internasional
Mematuhi peraturan perjalanan PBS, peraturan perjalanan PBS membatasi semua perjalanan domestik dan internasional yang disponsori sekolah hingga setidaknya 31 Juli 2022.
Jika Anda telah melakukan perjalanan internasional sejak 1 Februari 2021 (harus melampirkan bukti perjalanan), laporkan perjalanan internasional pada Formulir Perjalanan ini.
Menjaga Kesehatan diri sendiri dan karantina mandiri selama 14 hari.
Setelah selesai karantina, ajukan permintaan untuk kembali masuk Sekolah dengan mengisi Formulir.

Implementation of New COVID-19 Tracking Forms
Dear PBS Teachers & Administration,
COVID-19 is expected to impact our community for months to come. Although the pandemic remains a fluid situation, PBS’s commitment to the health and safety of our students, teachers, and staff remains as strong as ever. To safeguard our school community, PBS Covid-19 Management Team is implementing two new forms to track confirmed COVID-19 cases and possible COVID-19 exposures within the PBS community.
The PBS Community can access these documents located in the Forms Section of PBS’s COVID-19 website
Teachers, staff and students who have been diagnosed with COVID-19 must report the diagnosis to the school. Additionally, faculty, staff and students who believe that they have been exposed to an individual with a confirmed case of COVID-19 must also report that information to the school.
Once a form is completed, health officials will be notified, the information will be routed to the appropriate PBS personnel and the process of contact tracing will be initiated. Individuals who may have been exposed will be notified directly.
Those members of the PBS Community who are diagnosed or exposed to confirmed COVID-19 cases must adhere to the following School protocols:
Personal, Domestic and International Travel
Adhere to PBS’s expanded travel policy PBS’s expanded travel policy restricting all school-sponsored domestic and international travel through at least July 31, 2022.If you have travelled internationally since February 1, 2021 (to include cruises), then report international travel on this Travel Form. Self-monitor for symptoms and quarantine for 14 days.Upon completion of quarantine, submit a Request to Return to Campus Form.

Please keep your community and public health responsibility in mind. Actions everyone can take to mitigate the spread of infection include everyday preventive measures, such as social distancing, proper hand washing, wearing face coverings, and staying home when sick.
